Hydration

What to Do Next

  • Discomfort or burning when passing urine

  • Avoiding drinks to reduce toileting

  • Fatigue, confusion, or low appetite

  • Strong-smelling or dark urine

What to do

  2. Offer small, frequent drinks rather than large amounts

  3. Support toileting needs to reduce anxiety

  4. Follow the care plan and record observations

  5. Do not give medical advice, but note symptoms clearly

When to report

Report to the office if:

  • Symptoms of infection are noticed

  • Fluid intake is being deliberately restricted

  • Toileting changes are ongoing or worsening

  • The person appears unwell or more confused

Remember

  • Avoiding fluids can increase infection risk

  • Frequent toileting is not a reason to reduce hydration

  • Early reporting helps prevent deterioration
