Falls

What to Do Next

Use this guidance if someone appears unsteady, has had a recent fall, or you’re concerned about their safety when moving.

What to look out for

  • Holding onto furniture or walls

  • Shuffling, dizziness, or sudden weakness

  • Rushing to stand or move without support

  • Changes in confidence or fear of falling

What to do

  1. Pause and observe before leaving

  2. Encourage safe movement, using aids or support already in place

  3. Do not rush the person or the visit

  4. Reduce immediate hazards where safe to do so

  5. Follow the care plan and any falls guidance

When to report

Report to the office if:

  • Someone has fallen or nearly fallen

  • Their balance or confidence has changed

  • You are worried about leaving them safely

  • Equipment or support no longer seems adequate

If a fall occurs

  • Ensure the person is safe and comfortable

  • Do not move them unless trained and it is safe

  • Seek medical help if needed

  • Report the incident immediately and complete the required documentation

Remember

  • Falls often happen in familiar spaces

  • Small changes can signal increased risk

  • Trust your judgement, it’s better to raise a concern early
