What to Do Next

Use this guidance if you notice changes that may suggest someone is approaching the end of life.

What to look out for

  • Reduced appetite or fluid intake

  • Increased tiredness or sleeping more

  • Changes in breathing

  • Withdrawal or reduced communication

  • Increased need for comfort and reassurance


What to do

  1. Prioritise comfort and dignity

  2. Follow the care plan and any advance care wishes

  3. Speak calmly and reassuringly

  4. Observe and record changes clearly

  5. Contact the office if anything feels different or concerning


What not to do

  • Don’t make assumptions or give timelines

  • Don’t alarm the person or family

  • Don’t ignore changes because you’re unsure


When to report

Report to the office immediately if:

  • You notice significant changes

  • The person appears distressed or in pain

  • You feel unsure about what you’re seeing

  • Family members express concern


Remember

  • You are not expected to recognise end of life alone

  • Small observations can make a big difference

  • Support is available for you, the person, and their family
